Please take a moment to read this page as it will give you some information to make your first appointment as successful as possible. On this page you will our New Patient Forms for you to complete, condition specific forms, information about what to expect on your first visit, insurance accepted at our office and methods of payment.
Please download and print out the New Patient Intake Forms located in the button down below. See Additional Forms section for additional forms you may need to complete for your specific condition. Filling them out before your first visit will save time at the office.
If you are not able to fill out the forms before your first appointment, please arrive 30 to 45 minutes early for your first appointment.

Motor Vehicle Collision
If your condition is related to a car collision claim, please fill out this form. Please ask if you have any questions.

What to Expect
Expect the first visit and exam to last approximately 1 hour. All new patients will receive a complete history, review of systems and examination. Click here for more information on what to expect on your first visit. A full history and exam will be done for all new patients, regardless if they just want to get in for a quick adjustment. Especially complex cases may need multiple appointments to complete the initial history and examination.
If the doctor feels it is appropriate, you might have treatment on the first visit, to include chiropractic manipulation or other services. Your diagnosis will be explained to you as well as how it relates to your symptoms. If you have any questions, please ask.
If needed, diagnostic imaging or blood tests will be performed at a local facility.
Please be sure to bring your driver’s license, any primary or secondary insurance information, your payment and a calendar/appointment book with you.
If accurate insurance information is given over the phone, the fee charged will be more accurate for the first visit.
Harmony Chiropractic Center, Inc. accepts several types of insurance. For those who do not have chiropractic coverage on their insurance or those that wish to see Dr. Royer as an out-of-network doctor, the services are reasonably priced. Harmony Chiropractic Center, Inc. will contact your insurance company and find out your insurance benefits and communicate them to you.
Occasionally, the insurance companies that we accept have plans that have no chiropractic coverage
Please remember that verifying your insurance benefits is ultimately your responsibility. Occasionally, the information that we are given by the insurance company is not accurate so it is always good to double check.
Harmony Chiropractic Center, Inc. currently accepts the following insurance plans:
- Medical Mutual of Ohio
- FrontPath Health Coalition
- Meritain Health
- Ohio Bureau of Worker’s Compensation
- Automobile Insurance (in case of Motor Vehicle Collisions)
- Personal Injury Cases
Harmony Chiropractic Center, Inc. does NOT accept the following insurance plans:
- Anthem Blue Cross Blue Shield
- American Chiropractic Network (ACN)
- United Health Care
- Medicaid (except for Paramount Advantage)
- American Specialty Health Network (ASHN)
We will bill insurance for you but it is your responsibility to pay as you go if you have an insurance that we do not accept.
Certain insurances that we do accept have policies that do not cover chiropractic care.
Your payments may be made by cash, check or credit card. Visa, MasterCard, American Express and Discover accepted.
Please call the office at 419.517.5055 if you do not see your insurance listed above. You can also go to the website listed on your card to verify if Dr. Bryan D. Royer is in-network.
